SAMC/MWR Fishing Trip Registration Form 

All children of deployed soldiers in the age range of 6 to 15 are welcome to participate. Your child will be provided with an identification badge to let everyone know that they are with the SAMC/MWR fishing trip. There will be one adult for every six children throughout the duration of this event. We plan on assisting the children with fishing at LETRA, providing nutritious snacks and drinks while we are at the lake and some fun games for those who lose attraction in the fishing. Please ensure you bring this form, completely filled out, and any donations you would like to make with you to the drop off location. Donations made will support this function and other functions performed by the Fort Sill Sergeant Audie Murphy Club.

 

Your child may bring his/her own fishing equipment; we will provide some equipment.
